I have read and understand QMR's servioce agreement, FCRA requirments and FIco score handling abd agree to take all reasonable measures to enforce them within my facility.

I agree that I will use the QMR credit report for no other purpose other than what is stated in the permussable purpose section of the fair credit reporting act (fcra) section 604 (15USC 1681B) and as I stated on my QMR service agreement.

I understand and agree that I will not resell credit reports to any consumer or business directly or indirectly.

I plan to obtain FICO scores thur QMR from the colloring credit repositories I have selected below:

I understand and agree that the fico scores & reason codes received on my credit reports will be used only for the permissable purpose I selected on my QMR service agreement.

I Also understand that the scores & reason codes are for my use only and may not be transferred, distributed or resold to any third party. I further agree to maintain strict internal procedures to minimize the risk of unauthorized disclosure of scoring information to unauthorized parties. I agree to comply with all state and federal laws as they apply to credit scores.
